Strong gene disease association
Immunodeficiency presenting with high fever, cytopenias, and hepatosplenomegaly +/- liver dyfunction and neurological sx
Can present in infancy or in utero but variable age of onset including into adulthood - very severe death within 2 months of symptoms in children.
Treatment: etoposide-containing regimens such as the HLH-94 and HLH-2004 protocols followed by allogeneic hematopoietic stem cell transplantation (HSCT)
Outcomes better if performed pre-symptomatically.
Non genetic tests: Decreased expression of perforin, Decreased NK cell degranulation (the CD107a assay)
